- PHYSICAL FORM – ONLY ONE NEEDED FOR ALL 3 YEARS AT KMS

After a few parents reached out requesting information on our required sports forms, We wanted to make sure you are all informed and ready for the upcoming school year.
Sports Physical: Please upload this document to Rank One – either website or app is fine.
- One required for ALL years at KMS ( good from July before your 6th grade year, to May of your 8th grade year.
- Families should submit their physical directly to Rank One – not the school office.
- How to activate your RankONE account:
- Below is a step by step guide. You will need your student athletes' student ID number (found in PowerSchool) and their birthdate.
- Go to the Rank One login page
- Select Add Studen
- Enter your student’s Date of Birth and Student ID number
- Select Start Forms
- Upload your student’s physical and required participation documents
- This is a parent tutorial video is available to guide you through the process.

Concussion form: Complete on Parentsquare
- One copy required EACH school year.
- Paper Copy – not recommended

Not a full time student at KMS?
Thats OK, You can Dual Enroll your student!
A Dual Enrolled student is ANY student who is not at a KSD school full time. this includes any students who attend school for electives only.

- complete the “Athletics Checklist”
Dual Enrollment Procedure
Dual enrollment (DE) is offered to students from charter schools, homeschool programs and private schools. Students must be approved for dual enrollment before they can be enrolled at a district school and/or participate in sports/activities.
Process
- Complete Application: DUAL ENROLLMENT APPLICATION
- State law mandates that dual enrolled students MUST participate in sports and other competitive activities with their zoned school.
- District reviews app for complete information, verifies signatures and address, reviews space available for requested courses or activities, approves or denies the application, and notifies the school.
